REGISTRATION FORMS

This form is to be used for Faith Formation Programs: Summer, Sunday, or Home Study. Please download the form, complete and return to the Faith Formation Office with the registration fee. New students and transfer students must also complete a NEW Family Information Form.

Please remember to download and read the Faith Formation Guidelines for our classes. Also included are Summer Session Guidelines and Volunteer Forms. Please consider helping out either in the classroom, hall monitor, or as a snack aide.
Faith Formation Guidelines

Summer Registration will take place on:
Monday May 21st: 8:00AM TO 12:00 NOON - School Auditorium (Back parking lot entrance)

Tuesday May 22nd, Wednesday May 23rd, and Thursday May 24th: 9:00AM TO 12:00 NOON (Rectory office entrance 120 Hoboken Road)

Registration is limited and will be closed after this week.

With the exception of First grade, only those students currently enrolled in any St. Joseph’s Faith Formation Programs are eligible for the Summer Session.
